目的：观察电针疗法对局灶脑缺血再灌注（cerebral ischemia reperfusion, CIR）损伤大鼠海马区细胞焦亡相关蛋白酶半胱天冬酶-1（Caspase-1）的影响。方法：Longa线栓法构建大鼠脑缺血再灌注损伤模型。选取30只雄性SD大鼠随机分为假手术组、模型组和电针治疗组，每组各10只。电针治疗组取足三里、三阴交、尺泽、合谷四穴进行针刺，每次20分钟。Zea-Longa评分法评估各组大鼠神经功能。苏木精-伊红（hematoxylin-eosin, HE）染色和2,3,5-氯化三苯基四氮唑（triphenyltetrazolium chloride, TTC）染色法观察大鼠脑组织病理形态及脑梗死面积。实时荧光定量（Reverse Transcription-Polymerase Chain Reaction , RT-PCR）、免疫印迹法（Western blot,WB）检测大鼠脑组织海马区焦亡相关蛋白酶Caspase-1mRNA和蛋白表达水平。结果：与假手术组比较，模型组的神经行为学评分显著升高(P＜0.01)，出现明显的神经细胞损伤病理特征和白色脑梗死区，脑组织中Caspase-1的mRNA及蛋白水平显著上调(P＜0.01)。与模型组相比，电针治疗组显著减轻大鼠神经功能缺损和病理损伤特征 (P＜0.01)，白色区域脑梗死体积百分比明显缩小(P＜0.01)，海马区Caspase-1的转录水平和转录后水平显著降低(P＜0.05)。结论：电针疗法可能是通过拮抗Caspase-1介导的细胞焦亡的激活，保护脑缺血再灌注受损的神经细胞。
Objective: To study the effect of electroacupuncture (EA) on the expression of cell pyroptosis related Caspase-1 in hippocampus of focal cerebral ischemia-reperfusion (CI/ R) damage in rats. Methods: The CI/ R injury model was induced by middle cerebral artery occlusion with intraluminal thread. A total of thirty male Sprague Dawley rats were randomly divided into sham operation (sham), model and EA groups and ten in every group. EA (2/15 Hz, 2mA, 20min) was applied daily to Zusanli (ST 36), Sanyinjiao (SP 6), Chize(LU 5) and Hegu (LI 4) . Then behavioral neurological assessment of all rats in each group was performed with Zea-Longa score method after CI/ R injury. We also separately observed the neuronal damage and areas of cerebral infarct by HE staining and TTC staining method. The expression of Caspase-1 was measured by qPCR and western blot, respectively. Results: Compared with the sham group, the scores of model group were notably increased (P<0.01) and the brain tissue in model rats showed obviously neuronal injury and white infarction area, both up-regulated the mRNA and protein levels of Caspase-1(P<0.01). While compared with the model group, the neural deficit scores and neuron injury were relieved with significant and infarction area markedly shrank in EA group (P<0.01). Meanwhile, the expression of Caspase-1 at transcription and post- transcription levels in hippocampus of rats in EA group were significantly decreased than those in model group (P<0.05). Conclusion: The protective effect of electroacupuncture therapy on rats with cerebral ischemia-reperfusion injury may inhibit the activation of Caspase-1dependent pyroptosis.
